Auto populate text box based on checkboxes |

I am building a PDF fillable form and have run into a java script roadblock. The form is a simple purchase order with one exception. I need to select a series of "Q-Notes" that our vendors need to adhere to. Currently there are 11 different selections. A popup box is generated with the various check boxes and some basic definitions. My problem is, what java script syntax to I use to populate a text box to display on the form. I am VERY new to java script and got the current code off of a PDF scripting website. Any help you can offer would be appreciated.
